A Medicare Advantage Plan is another health coverage choice available as part of Medicare. Medicare Advantage plans, some -times called "Part C" or "MA" plans, are provided by private companies such as QHP. Beneficiaries, who join an MA plan, receive all of the PART A (hospital) and PART B (medical) coverage from the company and they also receive additional benefits such as vision, dental, and prescription drug coverage.

Information is also provided for members on how to request formulary exceptions and members rights to file appeals and grievances.

Print versions of the formulary are also available on line or by request through the Customer Service Dept.
